Elevate your clinical decision-making skills and gain a well-rounded understanding of essential orthopedic concepts with ICL360.
This Instructional Course Lecture provides an evidence-based review of evolving strategies for adult spinal deformity surgery, emphasizing patient-specific alignment targets, regional and segmental correction, and the appropriate use of minimally invasive, hybrid, and open reconstruction. It also addresses lower extremity compensation, frailty, proximal junctional biomechanics, and planning errors associated with mechanical failure and revision surgery.

Learning Objectives
- Interpret adult spine deformity as a three-dimensional disorder influenced by pelvic, hip, knee, and lower extremity compensation.
- Distinguish fixed spine deformity from flexible or compensatory malalignment via full-length and dynamic imaging studies.
- Individualize alignment goals based on pelvic morphology, patient age, frailty, spinal shape, and physiologic reserve.
- Distribute lumbar and thoracic spine correction across appropriate spinal regions rather than relying solely on global alignment thresholds.
- Anticipate the sagittal consequences of coronal correction, rod application, interbody placement, compression, and distraction.
- Select an upper instrumented vertebra and proximal landing region based on regional alignment, bone quality, muscle integrity, and junctional biomechanics.
- Determine when minimally invasive, hybrid, selective, or open reconstruction is most appropriate for adult spine deformity.
- Recognize planning and execution errors that contribute to proximal junctional failure, coronal imbalance, rod fracture, and revision surgery.
