Learning Objectives
- Differentiate confirmatory criteria for fracture-related infection from suggestive clinical, laboratory, imaging, microbiologic, and histopathologic findings.
- Develop a diagnostic sampling strategy that maximizes organism recovery while limiting contamination and false-negative results.
- Identify patient-, injury-, and treatment-related risk factors for fracture-related infection and apply preventive measures throughout the perioperative period.
- Select débridement and implant retention, implant exchange, or staged reconstruction based on infection timing, fracture stability, reduction quality, soft-tissue status, and host physiology.
- Integrate systemic and local antimicrobial therapy with surgical débridement, dead-space management, and fracture stabilization.
- Evaluate chronic fracture-related infections based on fracture union, implant status, disease extent, bone loss, soft-tissue coverage, and host fitness.
- Formulate reconstructive strategies for infected nonunion and chronic osteomyelitis while preserving or restoring limb function.
- Coordinate multidisciplinary care involving orthopaedic trauma, infectious disease, plastic surgery, vascular surgery, and rehabilitation specialists.
