This all-encompassing course goes beyond traditional learning by offering a complete 360° perspective on orthopedic topics. Explore additional resources, including full-text readings from the Journal of the American Academy of Orthopaedic Surgeons (JAAOS), technique videos from Orthopaedic Video Theater (OVT), and links to external literature and PubMed. Elevate your clinical decision-making skills and gain a well-rounded understanding of essential orthopedic concepts with ICL360.
This Instructional Course Lecture provides a detailed review of emerging joint-preservation strategies for cartilage disorders and medial compartment overload, including the mechanical implantable shock absorber knee system, patient-specific osteotomy planning, and contemporary cartilage resurfacing options. Through expert commentary and complex case presentations, the course emphasizes patient selection, biomechanical assessment, subchondral bone status, technical planning, and integration of alignment, meniscal, ligament, and cartilage procedures.

Learning Objectives
- Differentiate surface-based chondral defects, osteochondral lesions, unicompartmental overload, and established osteoarthritis in selecting joint-preservation treatment options.
- Identify appropriate indications and contraindications for the mechanical implantable shock absorber knee system in patients with medial compartment knee pain or osteoarthritis.
- Explain the biomechanical rationale for implantable load-sharing devices and distinguish load sharing from joint distraction.
- Apply patient-specific instrumentation principles to knee osteotomy planning, including biplanar correction, slope modifications, hinge protection, and tunnel or screw-trajectory avoidance.
- Compare microfracture, microfracture augmentation, minced autologous cartilage, juvenile particulated cartilage, viable cartilage allograft, matrix-induced autologous chondrocyte implantation, osteochondral autograft transfer, and osteochondral scaffold options.
- Determine whether subchondral bone involvement alters the treatment strategy from surface resurfacing to osteochondral reconstruction.
- Integrate alignment, meniscal status, ligament stability, slope, cartilage status, and patient goals into complex joint-preservation decision making.
- Anticipate technical pitfalls in combined cartilage, meniscus, ligament, osteotomy, and load-sharing procedures.
