Description: The revision ACL requires a complex surgical algorithm. In the revision setting, we need to consider what surgery was done previously, along with the potential reasons as to why the ACL may have "failed", including concomitant injuries and underlying soft tissue or bony anatomy problems that may affect a successful outcome.


Please join us for a thoughtful discussion on ACL re-injury and surgical concepts to consider in the revision setting. This webinar will focus on opinions and technical considerations from four of the leading experts in the field.

 

Learning Objectives:

1. Understand the technical challenges related to the previous ACL surgery and how this affects a revision ACL. How to evaluate bone loss from previous ACL surgery and when to consider one vs two stage revision.
2. Identify when to consider a lateral extra-articular procedure into a revision ACL.
3. Understand when an osteotomy is indicated in a revision ACL. How much malalignment is too much?
4. Understand the effects of concomitant injuries on ACL retears. What is the role of the meniscus, MCL and posterolateral corner?

Description: Many state medical boards require training in safe and effective alleviation of pain and optimal opioid stewardship. This update of a prior webinar series provides orthopedic surgery-specific evidence and strategies for safe and effective alleviation of pain. Part 2 addresses pain as an illness. For instance, more pain than expected, for longer than expected, during recovery from injury or surgery (delayed recovery trajectory). Illnesses defined by symptoms and signs rather than objective, experimental verification of pathophysiology (potential social constructions). And medicalization of aspects of human illness and commonplace somatization of distress. In a moderated, interactive, panel discussion, we invite you to review the evidence regarding the potential benefits and the potential harms of illnesses defined by pain rather than pathophysiology. There are important distinctions to be made that can help you and your patients identify the safest and most effective ways to get comfortable.

 

Learning Objectives: 

1. Recognize the difference between conditions rooted in experimentally verifiable pathophysiology and those defined by symptoms in the absence, or paucity, of measurable pathophysiology.
2. Identify delayed recovery trajectories and increase the chances of accurate diagnosis and treatment of contributing factors.
3. Strategize techniques for achieving candor and veracity regarding the patient’s condition, while maintaining a trusting relationship.

Description: Fractures of the femoral head typically occur after high-energy trauma. Radiographs and cross-sectional imaging are used to appropriately classify and identify associated injuries or morphologic characteristics that may influence management. Identification of the radiographic and clinical hallmarks of an irreducible variant is critical to optimizing the patient's outcome. Nonsurgical treatment can be used in the setting of small, infrafoveal femoral head fractures without hip instability. When surgical treatment is indicated, an anterior (Smith-Petersen), posterior (Kocher-Langenbeck), or surgical hip dislocation approach may be used depending on injury details and surgeon preference. The purpose of this review was to summarize current evidence on femoral head fractures regarding indications, variant patterns, surgical approaches, and outcomes.

Description: Many state medical boards require training in safe and effective alleviation of pain and optimal opioid stewardship. This update of a prior webinar series provides orthopedic surgery-specific evidence and strategies  for safe and effective alleviation of pain. Part 3 addresses alleviation of pain. The important distinction here is between specific and nonspecific effects of an intervention. Specific effects are due directly to alterations in the pathophysiology targeted by the treatment. Nonspecific effects are not related to changes in pathophysiology. Nonspecific effects include placebo and nocebo effects (meaning or context effects), regression to the mean, and the self-limiting course of many symptom episodes. When an intervention seems to alleviate pain for a much longer duration than it’s physiological effect, that is a long-term nonspecific effect that can be summarized as “accommodation.” To weigh the potential benefits against the potential harms of an intervention, it’s important to have experimental evidence establishing specific effects of the intervention, a measure of the magnitude of the specific effects, and a measure of the actual and potential harms including psychological and financial harms in addition to iatrogenic harms. A patient’s decision to use or not use specific intervention on offer for safe and effective alleviation of pain can be based on the evidence regarding specific and nonspecific effects of the intervention along with these actual and potential harms

1. Recognize the difference between specific and nonspecific effects of an intervention for alleviation of pain.
2. Consider long-term nonspecific effects and avoid crediting those to the intervention.
3. Strategize effective pain alleviation techniques based on the evidence for specific effects, their magnitude and duration, and the associated potential harms.
