This Instructional Course Lecture (ICL) explores the complexities of pediatric elbow trauma, offering advanced strategies and practical guidance for the management of challenging fracture patterns. This course consists of five expertly crafted lectures, which provide critical insight on advanced diagnostic techniques, surgical strategies, and best practices to optimize the outcomes of patients with a supracondylar humerus fracture, radial neck fracture, Monteggia injury, or other elbow trauma.
Pediatric elbow trauma presents unique challenges because of developmental anatomy, varied fracture patterns, and the need for tailored surgical approaches. This course emphasizes the importance of meticulous radiographic evaluation, careful surgical planning, and innovative techniques to address complex cases. Insight and actionable solutions are offered to enhance diagnostic accuracy, improve fixation stability, and reduce complications in this demanding patient population.

Learning Objectives
- Interpret key radiographic markers, including the Baumann angle, the anterior humeral line, and the radiocapitellar line, to accurately diagnose pediatric elbow fractures and assess alignment and stability.
- Identify developmental anatomy and ossification patterns of the pediatric elbow, differentiating normal findings from subtle fracture patterns to avoid misdiagnoses.
- Use advanced surgical strategies for atypical supracondylar humerus fractures, such as varus/valgus malunions and flexion-type fractures, including provisional pinning, open reduction techniques, and safe nerve handling.
- Perform minimally invasive and open reduction techniques for radial neck fractures, including the Metaizeau method, arthrographic-guided reduction, and open approaches, in patients in whom closed methods fail.
- Incorporate biomechanically optimized pinning configurations, including cross pinning and medial entry pins, to manage complex supracondylar humerus fractures with medial comminution, reverse obliquity, or proximal patterns while minimizing nerve injury risk.
- Diagnose and manage Monteggia fractures via an ulna-first approach, advanced imaging studies to assess radiocapitellar alignment, and osteotomy or ligament reconstruction for chronic cases.
- Recognize and manage physeal injuries and transphyseal separations, using arthrography and appropriate reduction techniques to prevent growth disturbances and optimize long-term outcomes.
