Course Description:

This course provides a comprehensive overview of the prevention, diagnosis, and management of periprosthetic joint infection (PJI) of the shoulder. Participants will review host optimization and perioperative strategies to reduce infection risk, as well as evidence-based approaches to evaluating and diagnosing the painful shoulder arthroplasty using established consensus criteria. The course will explore treatment options for both acute and chronic shoulder PJI, including medical and surgical decision-making across revision strategies. Reconstructive techniques following implant explantation will also be discussed, with emphasis on managing bone loss and soft-tissue deficiencies to optimize patient outcomes.

Learning Objectives:
At the conclusion of this course, learners should be able to:

  • Identify strategies to prevent shoulder PJI, including host optimization and perioperative prevention.
  • Apply evaluation and diagnostic strategies for shoulder PJI, including ICM criteria and interpretation of intraoperative cultures during revision arthroplasty.
  • Select appropriate treatment strategies for acute and chronic shoulder PJI, including antibiotics, debridement with implant retention, one- or two-stage revision, intraoperative adjuncts, postoperative management, and management of unexpected positive cultures.
  • Describe reconstructive options following shoulder explantation, addressing glenoid and humeral bone and soft-tissue defects in one- and two-stage revision settings.
